Medical History: Slightly elevated blood pressure, Arthritis, diabetes (well controlled). History of heart attack 15 years ago.
Social History: Wife passed away 4 months ago (she was the chef of the house). Lives alone. Retired. Lives in a senior apartment complex. Does have social gatherings weekly.
Anthropometrics: 5’8” 150#, 30# weight loss since his last visit 3 months ago. BP 145/95.
Activity level: Limited. Uses a walker. (1.3 activity level)
Appearance: Sunken eyes, frail in appearance; visible protruding clavicle bone, dentures.

Physical Activity: Limited due to arthritis.
Breakfast: English muffin with butter; 1 c black coffee.
